THE RUNAROUND: Why the System Fails Chronic Pain Patients

Most people begin their journey to address pain, dysfunction, or disease by visiting their primary care provider (PCP), whether a physician, nurse practitioner, or physician assistant. In today’s healthcare system, PCPs serve as the gatekeepers, managing access to specialists and diagnostic resources. However, when it comes to chronic pain or non-traumatic injuries, many PCPs have limited training in kinesiology (the study of human movement) and physical medicine.

Following standardized protocols and constrained by time, providers often prescribe NSAIDs or other medications to manage symptoms. If symptoms persist, patients may be referred to basic physical therapy or offered corticosteroid injections. Additional imaging like MRIs or CT scans is often delayed due to insurance protocols, requiring preliminary X-rays first.

Eventually, if pain and dysfunction continue, the patient may be referred to a specialist—who often repeats exams, orders advanced imaging, and may even recommend surgical intervention.

The truth is, the conventional medical model isn’t designed to diagnose or treat the root cause of chronic musculoskeletal pain. It’s designed to manage symptoms, not solve them.
